The short version: a shock absorber only damps motion, while a strut damps motion and is a structural part of the suspension that carries the spring and helps locate the wheel. What a shock absorber does
A shock absorber is a damper and nothing else. Inside it, a piston moves through hydraulic fluid, and the resistance of that fluid converts suspension motion into heat. It has no structural role: separate springs hold the vehicle's weight and separate control arms and links locate the wheel. Take a shock off a vehicle that uses them and the car still sits at ride height and the wheel is still held in place. It just bounces uncontrollably.
Because it isn't structural, replacing a shock is generally straightforward. Two mounting points, no spring under tension, and no change to the suspension geometry, which is why an alignment usually isn't required afterward.
You'll find shocks on the rear of many front-wheel-drive cars and crossovers, on both ends of a lot of trucks and body-on-frame SUVs, and on solid-axle setups generally.
What a strut does differently
A strut contains a damper too, but it also carries the coil spring on a seat welded to its body, and it forms part of the suspension's structure. It replaces the upper control arm on a MacPherson strut design, and it's part of the steering axis, meaning the whole assembly pivots when you steer. It carries vertical load, cornering load, and braking load.
That's why replacing a strut changes alignment. Camber and often caster are set at or influenced by the strut's position, so a strut replacement effectively resets those angles and an alignment afterward isn't an upsell, it's part of doing the job correctly.
It's also why the labor is different. The coil spring on a strut is under substantial tension, and separating it from the strut body requires a proper spring compressor. A compressed coil spring that slips is genuinely dangerous. The industry's answer is the loaded or quick strut assembly: a complete unit with the strut, spring, upper mount, and bearing preassembled. It costs more in parts and eliminates the spring compressor entirely, which usually makes it the better total-cost choice.

How do you know they're worn?
The bounce test is the classic and it still works for badly worn units: push down hard on one corner of the vehicle and let go. A healthy damper settles in about one rebound. If the corner keeps oscillating, that damper isn't controlling the spring. It's a crude test that catches the obvious failures and misses the gradual ones.
Look for leaking. A damper that's wet or has fluid streaked down its body has lost its seal and its damping with it, and that's an unambiguous replace. Also look for dented bodies, torn dust boots, and cracked or collapsed upper mounts.
The driving symptoms are the ones people actually notice: excessive nose dive under braking, the rear squatting under acceleration, body roll that feels floaty in corners, the front end continuing to bob after a bump, and a harsh clunk over expansion joints. And check the tires. Worn dampers let the tire skip rather than stay planted, and the classic result is cupping, a scalloped wear pattern around the tread that also makes the tires noisy.
When should they be replaced?
There's no fixed interval the way there is with a timing belt. Ranges of 50,000 to 100,000 miles get quoted commonly, and that's a reasonable planning window, but the actual answer is condition-based. Rough roads, heavy loads, towing, and hard driving all shorten it, and a damper degrades gradually enough that most people adapt to it without noticing.
Replace them in pairs across an axle. Mismatched damping side to side makes a vehicle behave inconsistently under braking and cornering, which is exactly when you want it predictable. Doing all four at once is common when the vehicle is high mileage and all four are original.
Getting them done matters more than it sounds. Dampers are what keep the tire in contact with the road, so worn ones lengthen stopping distances and reduce grip in emergency maneuvers, on top of ruining tires through cupping.
